【2024波蘭交換】波蘭學生簽證如何填寫

在【2024波蘭交換】申請學生簽證(D類簽證)step by step / 細節說明這篇文章中，我有寫到如何預約簽證，在預約好之後，就可以開始填資料啦!記得一個小時內要填寫完畢喔!!

1. Surname (s) (family name(s)) ⮕ 英文姓氏(同護照)
2. Surname (s) at birth (previously used surname (s)) ⮕ 舊姓氏(如果沒改過姓就同1.)
3. First name(s) ⮕ 英文名字(同護照)
4. Date of birth (year-month-day) ⮕ 生日(年-月-日)
5. Place of birth ⮕ 出生地(同護照)
6. Country of birth ⮕ 出生國家(同護照)
7. Current nationality/ies ⮕ 現在的國籍

Original nationality (nationality at birth) ⮕ 出生的國籍

8. Sex ⮕ 性別
9. Martial status ⮕ 婚姻狀況
10. In the case of minors: Surname, first name, address (if different from applicant’s) and nationality of parental authority/legal guardian ⮕ 申請人未成年的話，要填寫法定代理人的資料，不是的話則不用，但要記得勾選does not apply
11. National identity number, where applicable ⮕ 身分證字號
12. Type of travel document ⮕ 選Ordinary passport
13. Travel document’s number (series and number) ⮕ 護照號碼
14. Date of issue (year-month-day) ⮕ 護照發照日期(年-月-日)
15. Valid until (year-month-day) ⮕ 護照到期日期(年-月-日)
16. Issued by ⮕ 發照機關(同護照)
17. Applicant’s home address and e-mail address ⮕ 申請人地址及電子信箱

State:國家

State/province:州/省(縣市)

Place:地區(鄉鎮市區)

Postal code:郵遞區號

Address:地址(可以去郵局查詢，點這裡)

E-mail:電子信箱

Phone area code:電話區域碼

Phone number:電話號碼

18.Residence in a country other than the country of current nationality ⮕ 目前居住的國家是否與現在的國籍不同，有不同才需要填寫

19.Current occupation ⮕ 職業(學生就填Student, trainee)

20.Employer and employer’s address and phone number. For students, name and address of school ⮕ 填寫雇主資訊，如果是學生就填學校資訊(是在台灣就讀的學校喔!不是要去交換的外國學校)

21.Main purpose(s) of the journey ⮕ 旅行的主要目的，交換的話填study

22.Destination country ⮕ 目的地國家(系統已經設定波蘭了，不用填)

23.Member State of first entry ⮕ 第一個進入的申根國家(因為要轉機，我是填德國)

24.Number of entries requested ⮕ 保險起見填Multiple entries

25.Visa requested for ⮕ 簽證的天數(我是按照機票上的入境天數下去算，簽證也是按照機票日期給，有些國家的簽證只會給學期開始到結束而已)

26.National visas issued during the past five years ⮕ 過去五年否有拿過波蘭的長期簽證

27.Fingerprints collected previously for the purpose of applying for a Schengen visa ⮕ 過去五年有沒有因為申請申根簽證而提供過指紋

29.Intended date of arrival to the Republic of Poland (year-month-day) ⮕ 入境波蘭的日期(年-月-日)

30. Intended date of departure from the Republic of Poland (year-month-day) ⮕ 離開波蘭的日期(年-月-日)

31.32.Surname and first name of the inviting person(s) in the the Republic of Poland. If not applicable, name of hotel(s) or temporary accommodation(s) in the the Republic of Poland. ⮕ 在波蘭的邀請人資訊，如果沒有的話，就填寫暫時居住地資訊或者旅館等資訊(因為是交換學生，所以我就是選Company後填寫交換學校的資訊，可以參考17.的翻譯)

傳真資訊的部分因為交換的學校沒有，所以我留白，辦簽證時收件小姐沒有要求我補上

❗小提醒❗我寫這篇文章的時候發現，申請表格需要填的資料有時不太一樣，像我申請時有一欄是傳真資訊，但現在回到網頁看，沒有這個欄位。另外有時題目與題號的位置，不同時期的網頁會有些許差別，但其實大同小異!!

33.Cost of travelling and living during the applicant’s stay is covered ⮕ 在波蘭旅行與居住期間花費的金錢來源(我選by the applicant himself/herself)

36.Means of support during your stay ⮕ 這邊就是接33.填支付費用的方式(我選Cash和Credit cards)

34.Information on the work permit, certificate of entry of the application in the register of seasonal work applications, declaration of entrusting work to a foreigner or exemption from the obligation to have a work permit ⮕ 因為申請的是學生簽證選does not apply

35.Personal data of the EU or EEA citizen you depend on. This question should be answered only by family members of EU or EEA citizens. ⮕ 家庭成員是EU或EEA國家公民需要填寫，不是則記得勾選does not apply

填寫完資料之後，閱讀注意事項，勾選完同意就可以送出!

❗送出後會出現一組密碼，可以稍微記錄下來，因為要取消預約的話就是要用這組密碼取消~真的忘了也沒關係，同一組密碼會出現在申請表的最後一頁，找到這兩欄的資訊就可以去取消系統取消囉!

❗然後申請表記得下載下來列印，關掉網頁的話就再也找不到了(系統會寄送郵件到信箱，但我之前送出後隔了一天才收到郵件，以免萬一還是要先下載喔!)之後簽名+壓日期就大功告成啦!!祝大家辦簽證順順利利~~
